About the Role:

As an LPN with PDI, you’ll know your work is helping someone live a healthier, happier, and more independent life by providing essential care in a residential setting and working closely with the team to ensure health, safety, and well-being. This is a hands-on role that includes such things as:

• Administering medications, monitoring effectiveness, and ensuring safety.
• Implementing personalized health care plans and documenting progress.
• Assisting with daily activities like bathing, dressing, and meals.
• Assisting with transportation to places of interest and necessary appointments.
• Attending care meetings and contributing to Individualized Support Plans (ISPs).
• Training and mentoring new team members to deliver exceptional care.

Who We’re Looking For:

• A licensed LPN in good standing (TN or Nursing Compact state).
• CPR/First Aid/BLS certification (preferred)
• A valid driver’s license
• Experience working with individuals in a residential or medical care setting (preferred).
• A reliable and dependable team player ready to embrace challenges and opportunities.
• Someone with a big heart and a drive to make a difference.
