Summary of Position

Under the direct supervision of the Behavioral Health Director and indirect supervision of the Community Health Director, provides an evaluation of the social determinants of health for patients and families; social and mental wellness education; and assists medical providers in helping patients and families access resources, referrals, counseling, insurance, treatment, and management of patients.

Qualifications

  • Must have a master’s of social work degree from an accredited program.
  • Must have an active, permanent license in social work in the state of Tennessee
    Conversationally competent in Spanish (preferred)

  • Resolves patient problems and needs by utilizing multidisciplinary team strategies.
  • Maintains a safe and clean working environment by complying with procedures, rules, and regulations; calls for assistance from health care support personnel.
  • Protects patients and employees by adhering to crisis mental health protocols.
  • Maintains confidentiality of patient/family interactions

Job Duties

  • Read, agree to follow & sign Mental Wellness Policies and Procedures.
  • Read, agree to follow & sign Employee Policies and Procedures.
  • Field daily calls from patients regarding insurance enrollment, benefits.
  • Coordinate with insurance case management for complex patients
  • Follow up for Children’s Special Services (CSS) referrals/ status.
  • Follow up when applying for Erlanger financial aid, coordinate with CSS as needed.
  • Assist with social security disability applications and benefit information.
  • Provide housing and transportation resources/ referrals.
  • Make referrals for medicolegal partnerships for CPS case follow-ups.
  • Case management for children with behavioral health needs within pediatric and behavioral health clinics
    - Parenting classes (for managing childhood maladaptive behaviors such as ADHD,
    Autism, etc.)
    - Counseling for the child and family
    - Communicate with the preschool/school to collaborate on behavioral health needs
  • Individual and Family Counseling.
  • Advocate for patient compliance to foster better relationships and communication between families and pediatric and behavioral health providers.
  • Evaluate patients’ health needs and coordinate clinic visits when appropriate or offer encouragement and caregiving recommendations for the patient's care in the home. 
  • Initiate follow-up communication with English- and Spanish-speaking patients or caregivers after a clinic or Emergency Department visit to evaluate treatment and attend to additional questions or concerns.
  • Document patient encounters in Practice Fusion and Therapy Notes
  • Bill through Practice Fusion for patient encounters
  • Comply with HIPAA and OSHA standards
  • Other duties as assigned
