Job Summary:

In your pivotal role as Community Health Worker, you will provide peer-to-peer information, support, and education to patients working to manage health conditions and challenges. Additionally, through individual meetings, group events, and user-friendly materials, you will help patients maximize their self-care and make appropriate use of medical services and community resources; you will also model & exemplify good health habits. This role will be in collaboration with emergency medicine providers and social workers.

REQUIREMENTS
High School Diploma or GED
Community Health Worker training preferred (as evidenced by transcript or certificate); if no formal training, you will need to be open to completing the CHW Training Program of TN (provided thru VU School of Nursing at no cost to you). Must have reliable transportation to support off-site patient visits; mileage reimbursement will be provided.
